目的 探讨控制营养状态(CONUT)评分作为晚期胃癌一线化疗预后标志物的潜在应用。方法 回顾 性分析2013年1月至2018年12月苏州大学附属常州肿瘤医院肿瘤内科收治的123例接受一线化疗晚期胃癌患者 化疗前CONUT评分。根据受试者工作特征曲线(ROC曲线)将123例患者分为低控制营养状态评分(≤2分)组 和高控制营养状态评分(≥3分)组,评估CONUT评分与晚期胃癌患者生存之间的关系。结果 生存分析 (Kaplan-Meier)和秩和检验(log-rank)显示高控制营养状态评分组患者的无进展生存期(PFS)(P=0.013) 和总生存期(OS)显著降低(P=0.01)。进一步的多因素分析表明,CONUT评分是影响患者PFS(HR= 1.575,95%CI1.095~2.267,P=0.014)和OS(HR=1.635,95%CI1.118~2.391,P=0.011)的独立因素。 结论 化疗前CONUT评分与晚期胃癌行一线化疗患者的肿瘤进展及不良预后相关,提示化疗前患者免疫营养 状况检测可以获得较好的预后信息。
Abstract:
Objective To investigate the potential value of controlling nutritional status(CONUT) score as a prognostic biomarker in patients with advanced gastric cancer treated with first-line chemo-therapy. Methods The CONUT scores of123 patients with advanced gastric cancer who received first-line chemotherapy in the Department of Oncology of Changzhou Cancer Hospital Affiliated to So-ochow University from January 2013 to December 2018 were retrospectively evaluated. According to re-ceiver operating characteristic (ROC)curve analysis, 123 patients were divided into two groups: low-CONUT group(≤2 points)and high-CONUT group(≥3 points), and the relationship between CO-NUT score and survival was evaluated.Results Kaplan-meier analysis and log-rank test showed that progression-free survival (PFS)(P=0.013)and overall survival(OS)were significantly reduced in the high-COONUT group(P=0.01). Further multivariate analysis showed that CONUT score was an independent factor influencing PFS(HR=1.575, 95%CI 1.095-2.267, P=0.014)and OS(HR= 1.635, 95%CI1.118-2.391,P=0.011). Conclusion The CONUT score before chemotherapy is asso-ciated with tumor progression and poor prognosis in patients with advanced gastric cancer who received first-line chemotherapy, suggesting that host immunonutritional status detection before chemotherapy can obtain better prognosis information.
