摘要:
抗中性粒细胞胞浆抗体(ANCA)相关血管炎(AAV)是一类可累及全身多系统的自身免疫性疾病,以 小血管炎和致病性ANCA存在为特点。包括肉芽肿性多血管炎(GPA)、显微镜下多血管炎(MPA)和嗜酸性 肉芽肿性多血管炎(EGPA),其中GPA主要与蛋白酶3(PR3)-ANCA相关,而MPA和EGPA主要与髓过氧 化物酶(MPO)-ANCA相关。AAV确切病因和发病机制尚未完全阐明,可能在某些遗传背景下由某些环境因 素所诱发。遗传易感性、ANCA、中性粒细胞外捕网途径、补体旁路途径、抗人溶酶体相关膜蛋白2抗体、血小 板以及体液免疫和细胞免疫等多种因素共同导致了这一疾病的发生。本研究就近年来关于AAV发病机制的研究 进展作简要综述。
