摘要:
目的 分析北京市顺义区不同地区肺癌死亡风险差异,探索居住地对肺癌死亡风险的影响。方法 收集 中国人口死亡信息登记管理系统和北京市死因监测信息系统报告的2008至2017年死亡数据,采用logistic回归模 型控制性别、年龄和受教育水平混杂,以木林镇、龙湾屯镇等预评估后风险较低的6个临近地区合并人群为对照 组,计算各地区较对照组的肺癌死亡风险比值比(OR)和95%CI。结果 21个研究地区中,共8个地区死于肺 癌风险显著高于对照组:顺义城区(OR=1.891,95%CI1.636~2.187)、天竺镇(OR=1.477,95%CI1.095~ 1.993)、空港街道(OR=1.476,95%CI1.079~2.019)、仁和镇(OR=1.229,95%CI1.033~1.464)、牛栏山镇 (OR=1.291, 95%CI1.063~1.569)、高丽营镇(OR=1.438,95%CI1.200~1.724)、南彩镇(OR=1.196,95% CI1.021~1.401)、和北小营镇(OR=1.194,95%CI1.01~1.412)。高风险地区主要集中在经济较发达的西部地 区,其中,经济水平最高的顺义城区死于肺癌风险最高,是对照组的1.891倍(P<0.0001);经济水平最低的北 小营镇死于肺癌风险最低,是对照组的1.194倍(P=0.0381)。结论 顺义区近10年肺癌死亡呈现经济水平高 地区肺癌死亡风险更高特征。
Abstract:
Objective To analyze the differences of lung cancer death risk in different Shunyi regions of Beijing for exploring the influence of residence place on lung cancer death risk. Methods The death data of China population death registration and management information system and Beijing mu-nicipal cause of death monitoring information system from 2008 to 2017 were collected. Logistic regres-sion was used to control the confounding factors such as gender, age and education level. The com-bined population in 6 areas with low economic development level in Shunyi district(Mulin town, Longwan town, etc)was used as the control group to calculate the Odds Ratio(OR)and 95% Confi-dence Interval(CI)of risk of lung cancer death in each observed area. Results Of the21 research regions, 8 regions had a significantly higher risk of death from lung cancer than that in the control group: Shunyi downtown(OR=1.891, 95%CI 1.636-2.187), Tianzhu town(OR=1.477, 95%CI 1.095-1.993), Konggang sub-district(OR=1.476, 95%CI1.079-2.019), Renhe town(OR=1.229, 95%CI1.033-1.464), Niulanshan town(OR=1.291, 95%CI1.063-1.569), Beixiaoying town(OR= 1.194, 95%CI1.01-1.412). Gaoliying town(OR=1.438, 95%CI1.200-1.724), Nancai town(OR= 1.196, 95%CI 1.021-1.401). The high-risk areas were mainly concentrated in the western more prosperous areas, among which, the Shunyi downtown with the highest economic level had the highest risk of dying from lung cancer, and was 1.891 times of risk in the control group(P<0.000 1). Beixi-aoying town with the lowest economic level had the lowest risk of dying from lung cancer, and was 1.194 times of risk in the control group(P=0.038 1). Conclusion In recent 10 years, lung cancer in Shunyi district showed a higher risk of death in the area with high economic level.
