摘要:

目的 上海市金山区石化社区自2018年初实行家庭医生责任制度，对比分析2018年度与2019年度精神 障碍类患者状况，为今后开展工作提供参考。方法 精神障碍类患者的资料通过日常入户（电话）随访调查收 集，主要包括服药、体检、社会活动参与以及生活质量等信息，并将该信息分为两组：2018年度组486例， 2019年度组491例。结果 2019年组实施家庭医生制度后患者面访率（70.36%）、体检率（91.82%）、社会参与 率（85.78）及免费服药规范管理率（100.00%）等指标均高于2018年组（分别为64.20%、87.21%、80.89%、 95.45%），且上述指标比较，差异均有统计学意义（P<0.05）。2019年组服药（不按时按量12.96%、拒药 17.90%、医嘱勿需服药13.58%等情况）和生活质量情况[生理（67.82±7.96）分、心理（68.56±8.13）分和社会 活动（69.12±0.92）分]均优于2018年组[8.35%、12.02%、20.57%、（70.91±8.54）分、（73.72±9.54）分、 （72.61±9.17）分]，差异均有统计学意义（P<0.05）。结论 社区精神障碍患者在社区家庭医生的指导下，精神 状况明显好转，社会参与能力有所提高，为今后更好的生活打下良好基础。

Abstract:

Objective To evaluate the effectiveness of family doctor responsibility system by comparing and analyzing the status of mental disorder patients between 2018 and 2019, and provide a reference for the future work. Methods The data of patients with mental disorders were collected through dai-ly household（telephone）follow-up survey, mainly including information on medication, physical exami-nation, participation in social activities and quality of life. These subjects were divided into 2018 group （486 cases）and 2019 group（491 cases）. Results After the implementation of the family doctor system, these indicators such as the face-to-face interview rate（70.36%）, the health examination rate （91.82%）, social participation rate（85.78%）, and free medication standard management rate （100.00%）in 2019 group were all significantly higher than the indicators in 2018 group（respectively: 64.20%,87.21%,80.89%,95.45%）, and there were statistically significant differences（all P<0.05）. The medication status（12.96% of patients without taking medicine on time and quantity,17.90% of patients refusing to take medicine, 13.58% of the patients who need not to take medicine）and life quality（physical 67.82±7.96, psychological 68.56±8.13, and social activities 69.12±0.92）of patients in 2019 group were better than those（respectively: 8.35%,12.02%,20.57%,70.91±8.54, 73.72±9.54, 72.61±9.17）in 2018 group, and the differences were statistically significant（P<0.05）. Conclusion With the guidance of community family doctors, the patients with mental disorders have improved sig-nificantly, and social participation capacity have improved, which will make a good foundation for abetter life in future.
