摘要:

目的 分析2005年与2015年福建省及分地市人群肝硬化疾病负担及变化趋势。方法 选用2015年全球 疾病负担研究结果，以2010年人口普查数据作为标准人口，计算福建省及分地市人群肝硬化患者标化死亡率、过 早死亡损失寿命年（YLL）、伤残损失寿命年（YLD）和伤残调整寿命年（DALY），分析2005年与2015年福建 省人群肝硬化的疾病负担。结果 2015年，福建省肝硬化导致的死亡人数为3767例，死亡率为1.47/10万，标 化死亡率为1.00/10万；因肝硬化导致DALY为9.96万人年，其中以YLL为主（97.98%），YLD为0.21万人年 （2.02%）；男性DALY为8.21万人年，女性为1.74万人年，男性DALY损失高于女性。2015年，福建省DALY 前三位依次为泉州市、福州市及漳州市；与2005年相比，除厦门市外，福建省分地市疾病负担DALY呈下降趋 势。结论 YLL是福建省肝硬化疾病负担的主要来源，45~64岁男性是肝硬化防治的重点人群。需针对肝硬化 重点地区进行防控，减轻肝硬化造成的损失，尤其是伤残所致的寿命损失。

Abstract:

Objective To analyze the status and trend of liver cirrhosis in2005 and 2015 in Fujian province. Methods Using2010 census data as the standard population, results of the Global Burden of Diseases Study 2015（GBD2015）were selected to calculate the standardized mortality rate, years of life lost（YLL）, years lived with disability（YLD）and disability adjusted life years（DALY）of patients with liver cirrhosis in Fujian province and its cities, and liver cirrhosis burden in Fujian prov-ince in 2005 and 2015 was analyzed.Results In 2015, a total of 3 676 people died of liver cirrho-sis, and the crude death rate of Fujian province was 1.47/100 000, and thestandardized death rate was 1.0/100 000. The YLD due to liver cirrhosis was 2 100 person years; the DALY due to liver cirrho-sis was 99 600 person years, of which YLL was the main one（97.98%）. The male DALY was 82 100 person years, and the female DALY was 17 400 person year, and the DALY in males was higher than that in females. In 2015, the top three cities of DALY in Fujian province were Quan-zhou, Zhangzhou and Fuzhou; compared with 2005, the DALY of the disease burden in the cities of Fujian province showed a downward trend, except for Xiamen.Conclusion YLL is the main source of liver cirrhosis disease in Fujian province. Men aged 45-64 years old are the key population for cir-rhosis prevention and treatment. It is necessary to prevent and control the key areas of liver cirrhosis and reduce the loss caused by liver liver cirrhosis, especially YLD.
