摘要:
目的 了解北京市朝阳区B群链球菌(GBS)耐药性及耐药基因和毒力基因的携带情况。方法 选取分 离于北京市朝阳区妇幼保健院2016年2—6月孕4~10周女性宫颈携带的29株GBS,使用微量肉汤稀释法和药敏 纸片琼脂扩散法(Kirby-Bauer法,K-B法)测定细菌耐药表型,PCR检测菌株耐药基因和毒力基因。结果 29株GBS对青霉素、美罗培南、头孢噻肟、万古霉素敏感率均为100.00%;对红霉素、四环素、克林霉素、左 氧氟沙星、加替沙星、氯霉素、利奈唑胺耐药率依次为82.76%、75.86%、44.83%、37.93%、37.93%、6.90%、 6.90%;29株GBS中检出含有耐药基因pbp1a 28株(96.55%)、pbp2b 26株(89.66%)、gyrB 19株 (65.52%)、linB15株(51.72%)、ermB14株(48.28%)、tetM13株(44.83%)、mefA11株(37.93%)、 tetK8株(27.59%)、tetO5株(17.24%);检出含有毒力基因lmb 22株(75.86%)、ScpB10株(34.48%)、 bca 13株(44.83%)、hylB10株(34.48%)、bac 8株(27.59%)。结论 北京市朝阳区GBS对青霉素尚未出现 耐药菌株,针对青霉素过敏患者不建议使用红霉素,可使用氯霉素、利奈唑胺等低耐药率的抗生素。lmb和bca 是GBS的主要的毒力基因。
Abstract:
Objective To understandthe status of drug resistance and drug resistance genes and viru-lence genes of group B streptococcusin (GBS)Chaoyang district, Beijing. Methods Totally 29 strains of GBS were chosen from cervix of female subjects with pregnancy for 4 to 10 weeks in Bei-jing Chaoyang Maternal and Child Health Hospital from February to June in 2016. Broth microdilution method and disc diffusion method(Kirby-Bauer method)were used to determine the Bacterial resis-tance phenotype. Resistance genes and virulence genes of bacteria were detected by PCR. Results The sensitivity rate of GBS was100.0% to pennicillin, meropenem, cefotaxime, vancomycin, respec-tively. The drug resistance rate of GBS to erythromycin, tetracycline, levofloxacin, clindamycin, gati-floxacin, chloramphenicol, Linezolid was 82.76%,75.86%,44.83%,37.93%,37.93%,6.90%,6.90%, respectively. Penicillin resistant genes were found from 29 strains of GBS, including 28 strains of pbp1a (96.55%), 26 strains of pbp2b(89.66%), 19 strains of gyrB(65.52%), 15 strains of linB (51.72%), 14 strains of ermB(48.28%), 13 strains of tetM (44.83%), 11 strains of mefA (37.93%), 8 strains of tetK(27.59%), 5 strains of tetO(17.24%). Totally 22 strains of toxingene such as lmb gene(75.86%), 10 strains of ScpB gene, 13 strains of bca gene, 10 strains of hylB gene(34.48%), 8 strains of bac gene(27.59%)were found in29 strains of GBS. Conclusion In Chaoyang district of Beijing, GBS has no resistant strains to penicillin, erythromycin is not recom-mended for patients with penicillin allergy, and antibiotics with low resistance rate such as chloramphen-icol and linezolid can be used. The lmb and bca are the main virulence genes of GBS.
