摘要:
目的 探讨血浆脂蛋白相关磷脂酶A2（Lp-PLA2）水平在冠心病患者严重程度评估中的应用价值。方法 选取南方医科大学附属佛山市第二人民医院2016年1月—2018年1月收治的80例冠心病患者纳入观察组，并根据 WHO冠心病分型标准分为三个亚组，其中稳定型心绞痛20例、不稳定型心绞痛29例、心肌梗死31例。另选同 期检查未见冠状动脉狭窄的80例患者纳入对照组。使用酶联免疫吸附实验（ELISA）法测定血清中血浆Lp-PLA2水平。结果 稳定型心绞痛组、不稳定型心绞痛组、心肌梗死组血浆Lp-PLA2水平显著高于对照组 （P<0.05）。稳定型心绞痛组血浆Lp-PLA2水平最低，其次是不稳定型心绞痛组，心肌梗死组血浆Lp-PLA2水 平最高。单支病变组、双支病变组、三支病变组血浆Lp-PLA2水平显著高于对照组（P<0.05），单支病变组、 双支病变组、三支病变组间差异有统计学意义（P<0.05）。Gensin0~19分、20~40分、≥40分血浆Lp-PLA2水 平显著高于对照组（P<0.05），0~19分、20~40分、≥40分组间比较差异有统计学意义（P<0.05）。结论 血 浆Lp-PLA2水平可影响冠心病严重程度、病变血管支数，临床检测有利于病情严重程度评估。
Abstract:
Objective To investigate the predictive value of lipoprotein-associated phospholipase A2 （Lp-PLA2）on the severity of patients with coronary artery disease（CAD）. Methods Totally80 CAD patients（observation group）and 80 non-CAD patients（control group）selected from Second People’s Hospital of Foshan from January 2016 to January 2018 were enrolled. The observation group was further divided into three subgroups according to the WHO classification criteria of CAD, namely, stable angina pectoris group（n=20）, unstable angina pectoris group（n=29）, myocardial infarction group（n=31）. Plasma Lp-PLA2 level of the selected subjects was detected by the ELISA. Results Plasma Lp-PLA2 levels in observation group were higher than those in control group, and the ranking of plasma Lp-PLA2 levels was in an ascending order in stable angina pectoris group, unstable angina pectoris group and myocardial infarction group, with statistic difference（all P<0.05）. Plasma Lp-PLA2 levels in single-branch group, double-branch group and three-branch group were significantly higher than those in control group, and with statistic difference among the three groups（P<0.05）. The ranking of plasma Lp-PLA2 levels in control group, Gensini 0~20 score group，0~20 score group, 20~40 score group, and ≥40 score group was in an ascending order, with statistic difference （all P<0.05）. Conclusions Plasma Lp-PLA2 can influence severity and number of vessel branchesof CAD, so it can be used as a predictor for evaluating the degree of severity in CAD patients.
