摘要:
目的采用空腹血糖(FPG) 联合糖尿病风险量表的方法在社区人群中进行糖尿病早期筛查,探究高效 的筛查方式,达到降低糖尿病前期(IGR) 人群向糖尿病转化目的。方法采用整群随机抽样方法,选取 2014 年1 月—2018 年1 月山东黄河医院所辖社区的56 718 名居民健康档案,按照糖尿病高危人群筛查标准,对 45 岁及以上符合标准的居民采用FPG 联合丹麦糖尿病危险因素(DDRS) 量表对无症状糖尿病高危人群进行筛 查,并对糖尿病高危人群危险因素进行分析,探究筛查方法的效果,以山东省居民健康档案相关体检中血糖数值 为基础, 将患者分为正常血糖(NGR)、IGR 和糖尿病(DM) 3 类。结果2 型糖尿病(T2DM) 940 例 (20.08%),女性高于男性,≥65 岁年龄组为所有年龄组中最高(9.21%),差异有统计学意义(P <0.05),IGR 为 1 026例(21.91%),不同性别比较无统计学差异(P>0.05);IGR 组中腹型肥胖和血压异常检出率高于其他危险 因素检出率,组内比较差异有统计学意义(P <0.05);T2DM 组中IGT 或IFG 史的检出率高,与其他危险因素比 较差异有统计学意义(P <0.01);FPG 联合DDRS 量表筛查糖尿病及IGR 敏感度高。FPB 联合DDRS 的方法在≥ 29 的切点取得最大灵敏度74.57%,特异度82.32%。结论FPG 联合DDRS 量表可以作为社区筛查糖尿病高危 人群的有效方法
Abstract:
Objective To observe the effect of the fasting blood glucose (FPG) combined with Danish diabetes risk scale (DDRS) method on early screening of diabetes in community population, and to explore the efficient way to reduce the risk of prediabetes (IGR) transformation to diabetes population. Methods Health records platform of 56 718 residents were collected using random sampling method, who were under the jurisdiction of Shandong Yellow River Hospital from January 2014 to January 2018. According to the screening criteria of diabetes in high- risk population, FPG combined with DDRS were used to screen asymptomatic diabetes high-risk population from aged 45 and above in accordance with the standards of the residents. High risk factors of diabetes were analyzed to explore the effect of screening methods. The patients were divided into normal blood glucose (NGR) , IGR and diabetes. (DM) according to FPG value of health records platform of community population in Shandong province. Results Totally 940 cases of T2DM (20.08% ) were found, female higher than male, ≥65 years old group was the highest among all age group, the difference was statistically signifi-cant (all P<0.05) . IGR was 1 026 cases (21.91%),and there was no significant difference in different sex group (P>0.05);abdominal obesity and dyslipidemia detection rate were higher than the other risk factors in IGR group, the difference was statistically significant (all P<0.05) . In T2DM group, IGT or IFG rate was significantly higher than other risk factors (all P<0.01) . The sensitivity of FPG combined with DDRS scales in screening diabetes and IGR was higher, and the maximum sensitivity was 74.57% , specificity was 82.32% at the point of more than or equal to 29. Conclusion FPG combined with DDRS method can be used as an effective method for screening high-risk population of diabetes mellitus in community.
