目的探讨心肾综合征患者早期血液动力学异常与内皮素水平的关系。方法随机选取2016 年7 月— 2017年6月饶平县人民医院诊治的70例心肾综合征患者,确诊后即刻监测肾功能的变化,检测血浆内皮素、血清 肌酐(Scr) 等水平,根据其内皮素水平分为高内皮素组(39例) 和低内皮素组(31例),并采用多普勒超声仪探 测左肾动脉搏动指数(PI)、阻力指数(RI)、最大血流速度(Vmax)、最小血流速度(Vmin) 和平均血流速度 (Vmean) 等,同时检测患者全血比高切黏度、全血比低切黏度、红细胞压积、血沉等血液流变学参数,比较早期 血液动力学异常与内皮素水平的关系。结果随着肾功能的恶化,不同分期心肾综合征患者血清内皮素、Scr均 逐渐升高,左肾动脉的血液动力学亦出现明显变化,差异有统计学意义(F=6.391,P<0.05)。高内皮素组患者 的血液流变学参数RI、Vmax、Vmean和Vmin增加,而PI 降低,与低内皮素组比较差异均有统计学意义(P<0.05)。 结论不同肾功能分期的心肾综合征患者存在明显的内皮素异常和血液动力学异常,两者存在相关性,检测内 皮素水平和肾血液动力学的变化有助于指导临床治疗。
Abstract:
Objective To evaluate the relationship between early hemodynamic abnormality and endothelin level in patients with heart and kidney syndrome. Methods Totally 70 patients with heart and kidney syndrome who were diagnosed and treated in the People’ s Hospital of Raoping from July 2016 to June 2017 were randomly selected. After diagnosis, the change of renal function of the patients were monitored, including the plasma endothelin, serum creatinine (Scr) etc. Doppler ultrasound instrument was used to detect the left renal artery pulsation index (PI) , resistance index (RI) , maximum blood flow velocity (Vmax) , minimal blood flow velocity (Vmin) and mean blood flow velocity (Vmean) . At the same time, the blood rheological parameters of whole blood viscosity at high shear, whole blood viscosity at low shear viscosity, hematocrit and erythrocyte sedimentation rate (ESR) were measured. The relationship between abnormal hemodynamics and endothelin level were analyzed. Results With deterioration of renal function, serum endothelin, Scr were increased, and the left renal artery hemodynamics was also significantly changed, the difference between groups with different stages was statistically significant (F=6.391, P<0.05) . Compared with the lower endothelin group, RI, Vmax, Vmean and Vmin were increased, PI was decreased in the high endothelin group, and the differences were statistically significant (P<0.05) . Conclusion The heart and kidney syndrome patientswith different renal insufficiency clinical stage have obvious abnormal endothelin levels and hemodynamics and they have significant correlation. Thus, detecting endothelin levels and renal hemodynamic changes will help to guide clinical treatment.
