摘要:
目的以脂肪细胞因子为靶标，探讨补肾活血法对绝经后骨质疏松症（PMO） 的干预效果，为临床治疗 提供参考。方法选择2017 年1—6 月广东中医药大学顺德医院骨科收治的120 例PMO 患者作为研究对象，按 随机数字表法分为观察组和对照组，各60 例。对照组患者口服碳酸钙D3 咀嚼片联合骨化三醇胶丸治疗，观察组 在对照组的基础上口服补肾活血方治疗，两组均连续治疗24 周。治疗前后，对照组进行VAS 疼痛评分，采用 双能量X 线吸收测量法检测两组腰1~腰4 椎体骨密度，采用ELISA 法检测两组血浆脂联素、瘦素，记录治疗过 程中不良反应的发生情况。结果治疗后，两组VAS 评分均较本组治疗前明显降低，腰1~腰4椎体骨密度均较 治疗前明显升高（均P<0.05），且组间差异有统计学意义（P<0.05）。治疗后，两组血浆脂联素和瘦素均较本组 治疗前明显下降（P<0.05），且观察组水平均明显低于对照组的，差异有统计学意义（P<0.05）。两组患者治疗 过程中均未见明显不良反应。结论补肾活血法辅助治疗PMO 能有效降低血浆脂联素、瘦素水平，影响骨代 谢，提高腰椎骨密度，缓解全身骨痛症状。
Abstract:
Abstract：Objective To investigate the intervention effect of Bushen Huoxue recipe on postmenopausal osteoporosis by adjusting adipocyte cytokines. Methods Totally 120 patients with postmenopausal osteoporosis from January 2017 to June 2017 in the department of Orthopedics in Shunde hospital were admitted and divided into the observation group （n=60） and the control group （n=60） . The control group were given oral calcium carbonate D3 chewable tablets combined with calcitriol capsules. The observation group were treated with Bushen Huoxue Decoction on the basis of the control group. Both groups had been treated for 24 weeks. VAS pain score were evaluated before treatment and 24 weeks after treatment. The vertebral bone mineral density of lumbar 1 - lumbar 4 were detected by dual-energy X- ray absorptiometry. Plasma adiponectin and leptin were measured by ELISA. The occurrence of adverse reactions in the course of treatment were recorded. Results The VAS scores of the observation group and the control group after treatment were significantly lower than those before treatment （P<0.05） , and the observation group was lower than the control group （P<0.05） . The vertebral bone mineral densities of lumbar 1-lumbar 4 of the observation group and the control group after treatment were significantly higher than those before treatment （P<0.05） , and the observation group washigher than the control group （P<0.05） .The levels of adiponectin and leptin in the observation group after treatment were lower than those before treatment and those in the control group （P<0.05） . Conclusion Bushen Huoxue recipe can effectively affect bone metabolism, improve lumbar bone mineral density and relieve the symptoms of generalized bone pain in the postmenopausal osteoporosis patients by reduceing the levels of adiponectin and leptin.
