摘要:

目的探讨甘肃省城乡50 岁及以上回族人群中白内障患病率、白内障影响因素、白内障覆盖率和社会负 担率。方法采用整群随机抽样方法，在甘肃省回族聚居地区城乡抽取50 岁及以上回族人群590 名进行眼部检 查，包括晶状体混浊程度和白内障手术情况。应用非条件多因素logistic回归分析城乡、性别、年龄、职业、文化 程度、收入、生活习惯和环境等因素对白内障患病率的影响。结果受检人群白内障患病率为48.47%，高龄、 文盲、农牧民人群白内障患病率明显增高，差异均有统计学意义（P＜0.05）。户外活动时间长、糖尿病病史是老 年性白内障危险因素，喝茶、身体锻炼习惯为保护因素。以双眼中较好的眼矫正视力小于0.05为标准，白内障手 术覆盖率为50.00%。白内障社会负担率为6.10%。结论在甘肃省城乡50岁及以上的回族人群中，白内障患病 率高于同年龄全国平均水平。白内障盲或重度视力损伤者造成的社会负担较重，手术覆盖率有待提高。

Abstract:

Objective To investigate the prevalence of cataract, the possible factors of cataract prevalence, cataract coverage and social burden rate in Hui populations aged 50 years and over in urban and rural areas of Gansu province. Methods The cluster random sampling method was used to collect 590 cases of Hui people aged 50 years and over from urban and rural areas in Gansu province, including degree of lens opacity and cataract surgery. Nonconditional multivariate logistic regression was used to analyze the effects of urban and rural, gender, age, occupation, education degree, income, living habits and environment on the prevalence of cataract. Results Among the subjects, the prevalence rate of cataract was 48.47%. The prevalence of cataract was significantly higher in the aged, illiterate, and herdsmen, and the differences were statistically significant （all P<0.05） . Outdoor activities for a long time, history of diabetes were all risk factors of senile cataract, but tea, physical exercise habits were protective factors. When based on the standard for less than 0.05 correcting vision with better eyes, the surgical coverage rate was 50.00%, and cataract social burden rate was 6.10%. Conclusion The prevalence of cataract is higher than the national average of the same age in Hui populations aged 50 years and over in Gansu. The social burden caused by cataract blindness and severe visual impairment are very heavy and cataract surgical coverage rate need to be improved.
