摘要:
目的探讨老年女性盆腔脓肿临床特点及处理方法。方法回顾性分析2006—2016 年在安徽省立 医院收治的16 例老年女性盆腔脓肿的临床资料，对其病史、临床表现、辅助检查、治疗方案进行统计与分 析。结果16 例病例中有盆腹腔手术史12 例；下腹痛14 例，盆腔包块15 例，发热5 例；白细胞增高7 例，B 超提示有包块15 例。进行手术治疗9 例，其中3 例合并阑尾疾病，2 例合并输卵管癌。16 例患者平 均住院时间15 d，其中15 例采用药物或手术对症治疗而痊愈出院，1 例包块减小家人要求出院，经随访患 者病情完全缓解。结论老年女性盆腔脓肿多伴有输卵管结扎、直肠、阑尾等其他盆腹腔手术史，临床表 现以发热、下腹痛、盆腔包块为主，首选抗生素保守治疗，7~10 d 无效时，应考虑输卵管癌或卵巢癌的 可能，尽早行剖腹探查以明确诊断，精确治疗。
Abstract:
Objective To study the clinical features and treatment of pelvic abscess in old female patients. Methods Retrospective analysis of 16 cases who were old women with pelvic abscess in Anhui Provincial Hospital from 2006 to 2016. The history,clinical manifestation,auxiliary examination and treatment plan were analyzed. Results There were 12 cases of pelvic abdominal surgery in 16 cases; 14 cases of abdominal pain, 15 cases of pelvic mass and 5 cases of fever. White blood cells increased in 7 cases, and ultrasound showed a mass in 15 cases. Surgical intervention was carried out in 9 cases, which 3 cases with disease of appendix and 2 cases with fallopian tube carcinoma. The average hospitalization time was 15 days in 16 patients, of which 15 cases were cured by drugs or surgery. The mass of the other case was decreased, while her families asked for discharing from hospital. The patient was followed up for complete remission. Conclusion Old female patients with pelvic abscess are often associated with tubal ligation, rectum, appendix, and other history of pelvic or abdominal operation. Clinical manifestations mainly with fever, abdominal pain and pelvic mass. The first choice is to use antibiotics as conservative treatment. Fallopian tube carcinoma or ovarian cancer should be taken into account if 7-10 days of antibiotic therapy is not effective. The early laparotomy ought to be performed to confirm the diagnosis and accurate treatment.
