摘要:
目的 分析超声内镜下不同病理类型消化道黏膜下肿物特点，提高消化道黏膜下肿物的临床诊断率。 方法 收集锦州医科大学附属第一医院经超声内镜检查且有病理诊断的412例黏膜下肿物患者的超声及病理资 料进行统计分析。结果病变分布部位：胃>食管>结肠>直肠>十二指肠>回盲部>贲门；病理类型：平滑肌瘤107例 （食管57例、胃34例、贲门4例、十二指肠1例、回盲部1例、结肠7例、直肠3例），间质瘤76例（胃40例、食管 16例、结肠12例、直肠5例、回盲部3例），脂肪瘤56例（直肠15例、胃12例、结肠11例、十二指肠8例、回盲 部7例、食管3例），异位胰腺21例（胃12例、十二指肠8例、回盲部1例），神经内分泌瘤113例（直肠50例、 结肠45例、胃7例、十二指肠7例、回盲部4例），囊肿16例（食管9例、胃3例、回盲部2例、贲门1例、结肠1例）， 其他23例；病变组织来源：黏膜肌层143例，黏膜下层131例，固有肌层115例，其他23例；超声内镜对不同病 理类型黏膜下肿物诊断准确率总体为84.95%（Kappa值为0.75，P<0.05）。结论 不同病理类型消化道黏膜下 肿物各具特点，且超声内镜对消化道黏膜下肿物有较高的诊断价值，在消化道黏膜下疾病诊疗中具有重要参考意义。
Abstract:
Objective To analyze the characteristics of different pathological types of submucosal tu-mors in the digestive tract under endoscopic ultrasonography, and to improve the clinical diagnosis rate of submucosal masses in the digestive tract. Methods The ultrasonic and pathological data of412 pa-tients with submucosal tumors were collected and analyzed. Results Pathological distribution was stomach > esophagus > colon > rectum > duodenum > ileocecum > cardia. Pathological types includ-ed: leiomyoma in 107 cases （esophagus 53.27% , stomach31.78%）, stromal tumor in 76 cases （stomach 52.63%）, lipoma in 56 cases（rectum 26.78%, stomach21.43%）, heterotopic pancreas in 21 cases（stomach 57.14%, duodenum38.10%）, neuroendocrine tumor in 113 cases（rectum 44.25%, colon 39.83%）. There were 16 cases of cyst（56.25%）and 23 other cases. In terms of source of pathological tissue, there were 143 cases of muscularis mucosa, 131 cases of submucosa, 115 cases of muscularis propria and23 other cases. The diagnostic accuracy of endoscopic ultrasonography for differ-ent pathological types of submucous tumors was 84.95%（kappa value was 0.75,P<0.05）. Conclusions Different pathological types of submucosal tumors of the digestive tract have their own characteristics, and endoscopic ultrasonography has a high diagnostic value for submucosal tumors of the digestive tract, which has an important reference significance in the diagnosis and treatment of submucosal diseas-es of the digestive tract.
