目的 基于大数据分析不同年龄对儿童住院费用的差异性并提出年龄优化分组的合理化建议。方法 选 取2018年1—12月福棠儿童医学发展研究中心22家医院经过中国版疾病诊断相关分组(CN-DRG)分组器 分组为“<17岁”的414571例患儿作为研究对象,基于统计分析确定最优年龄分组节点。结果 综合儿童年 龄的生长周期变化及国内外多项研究结果,本研究初步选定4个年龄组别,对4种不同年龄分组方式进行评价发 现,以6岁为年龄节点对“<17岁”的DRG组进行优化细分,年龄?6岁组与7~17岁组内的住院费用同质性较 高,且年龄?6岁组与7~17岁组的平均住院费用比较,差异有统计学意义(P<0.05),分组结果合理性最优。 结论 结合儿童住院的实际情况,建议将儿童“<17岁”组进行调整。
Abstract:
Objective To analyze the differences in hospitalization costs for children at different ages and to make rational recommendations for age-optimized groupings based on big data analysis. Methods A total of414 571 inpatients from 22 hospitals in the Futang Children’s Medical Development Re-search Center were selected as subjects in 2018 according to the Chinese version of Diagnosis Related Groups(CN-DRG). This study would determine the best age grouping node based on statistical anal-ysis. Results Based on the changes in the growth cycle of children’s age and multiple research re-sults, weinitially selected 4 age groups and did assessment on them. Taking 6 years old as the node to optimize and subdivide the "<17 years" DRG group was the best choice because the hospitalization expenses of the 6-year-old group and the7 to 17-year-old group were more homogeneous, and the difference in average hospitalization expenses between the two groups was statistically significant. Conclusion Based on the actual situation of children’s hospitalization, we recommended to adjust the "<17 years old" groups.
