摘要:
目的 探讨无创正压通气(NIPPV)治疗急性心衰伴射血分数降低患者的临床效果。方法 选取2017 年9月至2020年1月新疆维吾尔自治区人民医院收治的、使用脉搏指示连续心排血量监测系统(PiCCO)的60例 急性心衰伴射血分数降低的住院患者,随机分为持续正压通气(CPAP)组和双水平正压通气(BiPAP)组,每 组30例。在常规治疗的基础上分别给予CPAP和BiPAP无创通气。治疗前后,检测并比较两组呼吸频率 (RR)、氧分压(PaO2)、二氧化碳分压(PaCO2),PiCCO监测参数[心率(HR)、平均动脉压(MAP)、每搏输 出量指数(SVI)、左心室收缩力指数(dPmax)]。结果 治疗后,两组患者RR和PaCO2低于本组治疗前, PaO2高于治疗前,且组间PaO2和PaCO2比较,差异均有统计学意义(P<0.05)。治疗后,两组患者SVI较治疗 前升高,HR、MAP降低,差异均有统计学意义(P<0.05)。结论 CPAP和BiPAP无创通气治疗急性心衰伴 射血分数降低患者有效,且BiPAP无创通气更有助于改善患者呼吸功能。
Abstract:
Objective To explore the clinical effect of non-invasive positive pressure ventilation(NIP-PV)in the treatment of acute left heart failure combined with reduced ejection fraction. Methods Totally60 patients with acute heart failure and reduced ejection fraction who used the PiCCO monitoring system from September 2017 to January 2020 were selected as the research subjects and randomlydi-vided into the continuous positive airway pressure(CPAP)group(n=30)and bilevel positive airway pressure(BiPAP)group(n=30). On the basis of conventional treatment, the above patients were giv-en CPAP and BiPAP, respectively. Respiratory rate(RR), partial pressure of oxygen(PaO2), partial pressure of carbon dioxide(PaCO2), and PiCCO monitoring parameters [heart rate(HR), mean arteri-al pressure(MAP), stroke output index(SVI), left ventricular contractility index(dPmax)] were measured before and after treatment.Results After treatment, the levels of RR and PaCO2 were de-creased in the two groups, and levels of PaO2 were increased, and there were significant differences in PaO2 and PaCO2 between the two groups(P<0.05). After treatment, the levels of SVI in the two groups were increased, and the levels of HR and MAP were decreased, and the differences were sta-tistically significant (P<0.05). Conclusion Both CPAP and BiPAP are effective in treating patients with acute heart failure and reduced ejection fraction, and BiPAP is more helpful for improving pa-tients’respiratory function.
