摘要:
目的 探讨北京市朝阳区大气PM2.5污染对医院门诊量的影响。方法 收集首都医科大学附属北京朝阳 医院2013年门诊数据,同时收集北京市朝阳区日均可吸入颗粒物(PM10)、细颗粒物(PM2.5)、二氧化硫 (SO2)、氮氧化物(NO2)浓度和日均气温、相对湿度数据。使用Generalized Additive Model(GAM)分析 PM2.5与门诊量的关系。结果 PM2.5对医院总门诊量影响比较,差异无统计学意义(P>0.05),对呼吸系统疾病 门诊量和循环系统疾病门诊量影响比较,差异有统计学意义(P<0.05);滞后2 d的PM2.5浓度每升高10μg/m 3, 呼吸系统疾病就诊人次的相对危险度(RR)值为1.002 4(95%CI1.001 0~1.003 8);滞后3 d的PM2.5浓度每 升高10 μg/m 3,循环系统疾病就诊人次的RR值为0.9987(95%CI0.997 7~0.999 7)。结论 北京市朝阳区 PM2.5浓度升高可能引起医院呼吸系统疾病门诊量增加、引起循环系统疾病门诊量减少。
Abstract:
Objective To explore the effect of atmospheric PM2.5 pollution on the outpatient volume of hospitals in Chao-yang district, Beijing. Methods Outpatient data of Beijing Chao-yang Hospital, Capital Medical University in2013 and data of daily average Particulate Matter 10(PM10), Particu-late Matter 2.5(PM2.5), sulfur dioxide(SO2), nitrogen dioxide(NO2), concentrations, daily aver-age temperature and relative humidity in Chao-yang district of Beijing at the same time were collected. The relationship between PM2.5 and outpatient volume wasanalyzed using Generalized Additive Model (GAM). Results There was no statistically significant difference in the influence of PM2.5 on the to-tal outpatient volume of hospitals(P>0.05). The differences of the influence of PM2.5 on the outpa-tient volume of respiratory diseases and the outpatient volume of circulatory diseases were statistically significant(P<0.05). As an increase of 10 μg/m 3 of PM2.5 concentration after 2 days lag, the RR val-ue of number of visits patients for respiratory diseases was 1.002 4(95%CI1.001 0-1.003 8). As an increase of 10 μg/m 3 of PM2.5 concentration after 3 days lag, the RR value of number of visits pa-tients for circulatory disease was 0.998 7(95%CI0.997 7-0.999 7). Conclusion The increase of PM2.5 concentration in Chaoyang district of Beijing may lead to an increase in outpatient volume for re-spiratory diseases and a decrease in outpatient volume for circulatory diseases.
