摘要:

目的 探讨新Bobath技术联合激痛点针刺疗法对偏瘫伴肩手综合征Ⅰ期患者上肢功能的疗效。方法 将48例偏瘫伴肩手综合征Ⅰ期患者随机分为两组，每组24例。对照组进行常规康复训练和激痛点针刺疗法，观 察组在常规康复训练和激痛点针刺疗法的基础上增加新Bobath技术。在治疗前和治疗4周后分别采用疼痛视觉模 拟评定法（VAS）、布氏分期（Brunnstrom分期）和简易肢体运动功能Fugl-Meyer（FMA）量化评测法评估患 者的上肢疼痛及运动功能。结果 治疗前，两组患者VAS评分、Brunnstrom分期和简易Fugl-Meyer评分比 较，差异无统计学意义（P>0.05）。治疗后，两组患者VAS评分显著降低（P<0.05）；Brunnstrom分期和简易 Fugl-Meyer评分较同组治疗前评分均显著升高（P<0.05），且观察组患者VAS评分较对照组降低显著、 Brunnstrom分期和简易Fugl-Meyer评分较对照组评分升高显著，差异均有统计学意义（P<0.05）。结论 新 Bobath技术联合激痛点针刺疗法可有效促进偏瘫伴肩手综合征Ⅰ期患者疼痛的减轻，且更有利于上肢功能的恢复。

Abstract:

Objective To investigate the effect of Bobath technique combined with acupressure point acu-puncture therapy on upper limb function in patients with hemiplegic accompanied with shoulder-hand syndrome in stageⅠ. Methods A total of48 cases of patients with Hemiplegia accompanied with shoulder-hand syndrome in stage I were randomly divided into treatment group and control group, 24 cases in each group. The control group received conventional rehabilitation training and stimulation pain point acupuncture therapy, and the treatment group received new Bobath technique on the basis of con-ventional rehabilitation training and stimulation pain point acupuncture therapy. Visual analogue scale （VAS）, Brunnstrom scale（Brunnstrom）and Fugl-Meyer（FMA）methods were used to assess upper limb pain and motor function before and 4 weeks after treatment.Results There were no significant difference in VAS score, Brunnstrom stage, and simple FMA score between the two groups before treat-ment（P>0.05）. After treatment, the VAS scores of the two groups were significantly reduced （P< 0.05）, Brunnstrom stage and simple FMA scores were significantly higher than those before treatment （P<0.05）. Compared with the control group, the VAS scores were significantly lower, Brunnstrom stag-ing, and simple FMA scores were significantly higher in the treatment group （P<0.05）. Conclusions The new Bobath technique combined with acupressure point acupuncture therapy can effectively relieve the pain of patients with hemiplegic accompanied with shoulder-hand syndrome, and is more beneficial tothe recovery of upper limb function.
