目的 探讨兰州市2013—2016年中老年肾病综合征（Nephrotic syndrome，NS）患者的住院费用构成及 其影响因素，为合理控制住院费用提供参考依据。方法 从兰州市卫生健康信息网收集2013—2016年中老年 NS患者的病案信息，采用灰色关联分析法和结构变动度分析法评价住院费用的影响因素及各单项费用的关联程 度和变动情况。采用混合线性模型评价住院费用的外部影响因素。结果 住院费用构成中，药品费是关联性最 大的项目，其次是诊断费；同时，药品费是影响住院费用结构变动的主要因素；年龄、住院天数、入院方式、是 否手术是患者住院费用的主要影响因素。结论 中老年NS患者住院费用结构不够合理，须优化调整各单项费用 结构。医院应缩短无效住院天数，从而减轻患者的住院费用。
Abstract:
Objective To explore the composition of hospitalization expenses and its influencing factors in patients with middle-aged and elderly patients with nephrotic syndrome in Lanzhou city from 2013 to 2016, and to provide basis for reasonable control of hospitalization expenses. Methods Medical re-cords of middle-aged and elderly patients with nephrotic syndrome from 2013 to 2016 were collected from Lanzhou health information network, and gray correlation and structural change degree were used to evaluate the influencing factors of hospitalization expenses and the degree of association and change of specific expense. A mixed linear model was used to evaluate the external factors of hospitalization costs. Results In the hospitalization expenses, the drug cost was the most relevant item, followed by the diagnosis fee. And the main cause of changes in the cost structure was affected by the drug fee. Age, hospitalization days, admission mode and whether or not surgery were the main influencing factors of patients’hospitalization expenses. Conclusions The hospitalization cost structure of middle-aged and elderly patients with nephrotic syndrome in Lanzhou city is not reasonable, so it is necessary to optimize and adjust the individual cost structure. Hospitals should shorten invalid hospitalization days, thus reducing the cost of hospitalization.
