目的 分析青年体检人群正常高值血压检出率,探讨其相关危险因素。方法 选取2017年1—8月在北 京市体检中心进行健康体检的青年9 064人,按现场血压检查结果分为正常血压组和正常高值血压组,对两组间 体质量指数(BMI)、腰围、总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)、血尿酸(UA)、血 肌酐(CRE)、血尿素氮(BUN)进行比较分析。结果 男性正常高值血压者2 295名(45.30%),高于女性的 1 209名(30.25%),差异有统计学意义(P=0.000)。正常血压组BMI、腰围、TC、TG、LDL-C、UA、 CRE、BUN水平均低于正常高值血压组,差异均有统计学意义(P<0.05)。性别分布、BMI、TG、LDL-C与正 常高值血压呈正相关;腰围、TC与正常高值血压呈负相关。根据logistic回归分析OR值,将BMI、TG、LDL-C进行ROC曲线分析,其中BMI的ROC曲线下面积值为0.632;Youden指数最大是0.198,对应的BMI为 23.75kg/m 2,即发生正常高值血压的临界值。结论 BMI>23.75kg/m 2的青年人群进行常规血压监测有利于早 期发现是否存在正常高值血压,男性更应该引起重视,而对于有血脂异常的人群需要更加积极的进行血压检查。
Abstract:
Objective To analyze the detection rate of normal high blood pressure and its related risk factors in young people. Methods From January to August in2017, the Beijing Physical Examination Center conducted a health checkup for9 064 young people. According to the level of blood pressure, they were divided into the normal blood pressure group and the normal high blood pressure group. Body mass index(BMI), waist circumference, total cholesterol(TC), triglyceride(TG), low-density lipo-protein cholesterol(LDL-C), blood uric acid(UA), serum creatinine(CRE), and blood urea nitro-gen(BUN)were compared and analyzed between the two groups.Results There were2 295 male cases (45.30%)and 1 209 female cases(30.25%)with normal high blood pressure, and there was sig-nificant difference in the detection rate between them(P=0.000). Levels of BMI, waist circumference, TC, TG, LDL-C, UA, CRE, and BUN in the normal blood pressure group were lower than those in the normal high blood pressure group(all P<0.05). Gender, BMI, TG, and LDL-C were positively related with normal high blood pressure(all P<0.05); waist circumference and TC were negatively relat-ed with normal high blood pressure(all P<0.05). Based on logistic regression analysis odds ratio, BMI, TG, and LDL-C were analyzed by ROC curve. BMI had the area of 0.632 under the ROC curve, and the maximum Youden index is 0.198. Its corresponding BMI was 23.75 kg/m 2 , which was the threshold when normal high blood pressure occured. Conclusion The routine monitoring of bloodpressure in young people with a BMI >23.75 kg/m 2 is conducive to early detection of normal high blood pressure. Men should be paid more attention to it. People with dyslipidemia need to be more active in blood pressure test.
