摘要:
目的 探讨X线平片下C7斜率与T1斜率的相关性及其与颈椎间盘退变的关系。方法 回顾性分析 2019年1—6月福建医科大学附属龙岩第一医院接受颈椎侧位X线平片和MRI检查的80例颈椎退行性疾病患者, 测量X线平片下C7斜率与T1斜率,参照MRI椎间盘退变Pfirrmann分级评分系统判定退变严重程度。结果 本 组患者X线平片下C7斜率为23.06°±7.84°,T1斜率为27.08°±7.42°。Person相关分析显示,C7斜率与T1斜率呈正 相关(r=0.917,P<0.001)。T1斜率关于C7斜率的直线回归方程为:T1斜率=7.09+0.87×C7斜率。和高C7斜 率组(>20°)比较,低C7斜率组(≤20°)各阶段颈椎间盘退变Pfirrmann分级总分较高[(13.94±1.63)vs. (12.63±1.30);t=4.001, P<0.001],中重度颈椎间盘退变(Pfirrmann分级评分≥15分)所占比例较高(40.62% vs. 8.33%,χ 2 =11.964,P=0.001)。低C7斜率组(≤20°)患者C5-6[(3.22±0.83) vs.(2.81±0.73);t=2.299, P=0.024]和C6-7[(2.78±0.61)vs.(2.42±0.58);t=2.708,P=0.008]节段椎间盘退变Pfirrmann分级评分显著大 于高C7斜率组(>20°),差异有统计学意义(P<0.05)。结论 X线平片下C7斜率与T1斜率存在线性正相关,低 C7斜率对预测C5-6和C6-7节段颈椎间盘退变具有价值,C7斜率可以作为X线平片下T1上终板显示不清而无法测量 T1斜率时的替代参数。
Abstract:
Objective To investigate the correlation between C7 slope and T1 slope on X-ray plain film and the relationship between C7 slope and cervical disc degeneration. Methods A total of80 pa-tients with cervical degenerative diseases who received lateral X-ray and MRI examination from Janu-ary 2019 to June 2019 in Longyan First Hospital Affiliated to Fujian Medical University were analyzed retrospectively. The C7 slope and T1 slope were measured under X-ray plain film, and the severity of degeneration was determined according to Pfirrmann grading system of MRI Intervertebral disc degenera-tion. Results The slopes of C7 and T1 under the X-ray plain film were 23.06°±7.84° and 27.08°± 7.42°, respectively. Pearson correlation analysis showed that C7 slope was positively correlated with T1 slope(r=0.917, P<0.001). The linear regression equation of T1 slope with respect to C7 slope is T1 slope=7.09+0.87×C7 slope. Compared with high C7 slope group(>20°), the total Pfirrmann score of cervical disc degeneration in low C7 slope group(≤20°)was significantly higher(13.94±1.63 vs 12.63± 1.30, t=4.001, P<0.001). The proportion of moderate and severe cervical disc degeneration (Pfir-rmann score ≥15)in low C7 slope group was significantly higher(40.62% vs8.33%,χ 2 =11.964, P=0.001). The Pfirrmann score of C5-6 (3.22±0.83 vs 2.81±0.73, t=2.299, P=0.024)and C6-7 (2.78± 0.61 vs 2.42±0.58, t=2.708, P=0.008)in low C7 slope group were significantly higher than those of high C7 slope group(P<0.05). Conclusion There is a linear positive correlation between C7 slope and T1 slope under X-ray plain film. Low C7 slope is valuable for predicting cervical disc degeneration at C5-6 and C6-7 segments. C7 slope can be used as an alternative parameter when T1 slope cannot be measured due to unclear display on T1 upper end plate under X-ray plain film.
