摘要:
目的探讨高同型半胱氨酸水平对老年冠心病患者肾功能的影响。方法选取2017年1月至2018年8月 民航总医院老年科收治的81例老年冠心病患者作为研究对象,按血浆同型半胱氨酸(Hcy) 水平分为高同型半胱 氨酸血症(HHcy) 组(38例) 和非高同型半胱氨酸血症组即对照组(43例)。测定两组患者血浆Hcy、空腹血糖 (FBG)、血清总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、丙氨酸氨基 转移酶(ALT)、B 型脑钠肽(BNP)、血清肌酐(Cr)、血清尿酸(UA)、糖化血红蛋白(HbA1c) 水平,计算肾 小球滤过率估计值(eGFR) 等。结果两组患者FBG、TC、HDL-C、LDL-C、ALT、BNP、HbA1c水平 差异无统计学意义(P>0.05)。HHcy 组患者Hcy、Cr、UA 水平均明显高于对照组,eGFR 明显低于对照组, 差异均有统计学意义(P<0.05); 血浆Hcy 水平与血清UA、Cr 水平均呈正相关(r=0.358、0.506, P= 0.001、<0.001),与eGFR 呈负相关(r=-0.444,P<0.001)。结论老年冠心病患者人群中,血浆Hcy 水平 可提示肾功能情况。
Abstract:
Objective To explore the effect of hyperhomocysteine (HHcy) level on renal function in the elderly patients with coronary artery disease. Methods A total of 81 cases of elderly patients with coronary artery disease admitted to Civil Aviation General Hospital from January 2017 to August 2018 were selected as the study subjects. According to plasma homocysteine level (Hcy) , they were divided into HHcy group (38 cases) and control group (43 cases) with normal Hcy. The plasma levels of Hcy, fasting blood glucose (FBG) , serum total cholesterol (TC) , high- density lipoprotein cholesterol (HDL- C) , low- density lipoprotein cholesterol (LDL- C) , alanine aminotransferase (ALT) , B-type brain natriuretic peptide (BNP) , serum creatinine (Cr) , serum uric acid (UA) , glycosylated hemoglobin (HbA1C) were measured, and glomerular filtration rate (eGFR) were calculated. Results The levels of FBG, TC, HDL-C, LDL-C, ALT, BNP, HbA1c had no statistical differences between HHcy group and control group (all P>0.05) . There were higher levels of Hcy, Cr, UA and lower GFR level in HHcy group compared with control group (all P<0.05) . The plasma Hcy level was positively correlated with serum UA, Cr (r=0.358, P=0.001; r=0.506, P= 0.001) , and the plasma Hcy levels was negatively correlated with eGFR level (r=- 0.444, P< 0.001;) .
