摘要:

目的探讨腰围身高比值（WHtR） 对预测社区自我管理2型糖尿病（T2DM） 患者血糖控制的影响，为 糖尿病高危人群的提早发现提供参考。方法收集整理上海市曹家渡社区2006—2016 年在册管理的678 例 T2DM 患者资料，采用单因素和Logistic多元回归分析患者建卡管理前的体质量指数（BMI）、腰围、WHtR、高 血压史和管理1年后空腹血糖控制情况的关系。结果不同性别、文化程度和是否有高血压病史患者的血糖控制 情况比较，差异无统计学意义（P>0.05）；不同年龄（进入社区管理时）、BMI、腰围、WHtR 的患血糖控制情况 比较，差异有统计学意义（P<0.05）。Logistic 多元回归分析发现，≥65 岁（RR=1.551，95%CI:1.037~2.322）、 超重/肥胖（RR=6.944，95%CI:4.292~11.236） 和WHtR>0.5 （RR=3.881，95%CI:2.282~6.600） 是社区糖尿 病患者血糖控制情况的危险因素（均P<0.05）。结论患者WHtR 是社区自我管理2型糖尿病患者血糖控制水平 的影响因素。

Abstract:

Objective To analyze the role of waist to height ratio （WHtR） in predicting the control conditions of blood glucose in patients with type 2 diabetes mellitus （T2DM） for early detection of high risk population of diabetes mellitus. Methods A total of 678 patients with T2DM on self-management in Caojiadu community of Shanghai from 2006 to 2016 were collected. The relationship of the control condition of blood glucose after 1 year with body mass index BMI） , waist, WHtR, hypertension history were analyzed by single factor and logistic regression analysis. Results There were no significant differences of blood glucose control conditions in different groups with different gender, education degree, hypertension history （all P>0.05） ; there were significant differences in different groups with different age, BMI, waist and WHtR （all P<0.05） . Logistic regression analysis showed that patients ≥65 years old （RR=1.551, 95% CI 1.037- 2.322） , overweight/obesity （RR=6.944, 95% CI 4.292-11.236） , and WHtR>0.5 （RR=3.881, 95%CI 2.282-6.600） were the risk factors of poor control condions of blood glucose for T2DM patients in community （P<0.05） . Conclusion WHtR has an important value in predicting the effect of self-management in T2DM patients.
