摘要:
目的系统评价太极拳对心力衰竭患者心功能及生活质量的影响。方法计算机检索PubMed、The Cochrane Library、EMBase、中国知网、维普数据库和万方数据库,由2 名研究者分别对纳入文献进行数据提 取和质量评价后,采用RevMan 5.3 软件进行meta 分析。结果从54 篇文献中选择纳入7 篇,共446 例患者参 与随机对照试验(RCT)。meta 分析结果显示:在心力衰竭患者康复过程中,与对照组相比,试验组患者LVEF 明显改善,差异有统计学意义(MD=8.38,95%CI:6.98~9.78,P<0.000 1);6 min 步行试验(6-MWT) 距 离明显升高,差异有统计学意义(SMD=0.85,95% CI:0.61~1.08,P<0.000 01);峰值摄氧量(Peak VO2) 的变化无明显差异(MD=1.03,95%CI:-1.71~3.78,P=0.46);明尼苏达生活质量调查评分(MLHFQ) 积分 改善更明显,差异有统计学意义(SMD=- 1.10,95% CI:- 1.91~- 0.29,P=0.008);N 前端脑钠肽(NTproBNP) 改善情况稍好,差异有统计学意义(MD=-12.14,95%CI:-23.78~-0.50,P=0.04)。结论meta 分析表明,练习太极拳可提高心力衰竭患者心功能,明显改善心力衰竭患者生活质量,适用于心力衰竭稳定期患 者的康复治疗。
Abstract:
Objective To systematically review the effect of Tai Chi on heart function and quality of life in patients with heart failure. Methods The databases of PubMed, Cochrane Library, EMBase, China CNKI, VIP database, and Wanfang were screened. A meta-analysis was then performed by using Rev Man 5.3 software after the included documents were extracted and evaluated by 2 researchers respectively. Results In this meta-analysis, seven randomized controlled trial (RCT) studies involving 446 subjects were conducted. The meta- analysis results showed that, compared with the control group, the LVEF of patients in the treatment group was improved and slightly higher during the rehabilitation process for heart failure,and this difference was statistically significant (MD=8.38, 95% CI 6.98-9.78, P<0.000 1) ; the 6 minute walk test (6-MWT) distance in the treatment group was obviously more than that in the control group, and it had statistical significance (SMD=0.85, 95%CI 0.61- 1.08, P<0.000 01) ; the change in Peak VO2 between the two groups had no significant difference (MD=1.03, 95% CI- 1.71- 3.78, P=0.46) ; the Minnesota Living with Heart Failure Questionnaire(MLHFQ) score was significantly better in the treatment group with the difference being statistically significant (SMD=-1.10, 95%CI -1.91--0.29, P=0.008) ; the N-terminal pro-brain natriuretic peptide (NT-proBNP) was also slightly better in the treatment group, and the difference was statistically significant (MD=-12.14, 95% CI -23.78--0.50, P=0.04) . Conclusion The meta-analysis showed that Tai Chi training can significantly improve the heart function and quality of life for the patientwith heart failure, and this treatment could be applied to the rehabilitation process of patients with stable heart failure.
