目的 了解福州市居民糖尿病死亡的原因分布,为制定糖尿病的防治对策提供依据。方法 采用入户调 查法,按照ICD—10的标准和方法进行死因分类,对福州市2011—2016年居民糖尿病死亡情况进行全面分析。 结果 2011—2016年福州市居民糖尿病死亡5 197例,平均死亡率为17.52/10万,其中女性高于男性(18.77/ 10万vs. 16.33/10万,Z=5.019,P<0.001),城市高于农村(28.75/10万vs. 10.43/10万,Z=36.719,P< 0.001)。45岁以上人群是糖尿病死亡率高发人群。居民减寿率为0.39‰、潜在工作损失率0.60‰,糖尿病对男性 患者潜在寿命损失(0.43‰vs.0.65‰)和潜在工作损失(0.36‰vs.0.54‰)的影响大于女性患者,差异均有 统计学意义(Z=9.652,12.229,P<0.001)。结论 糖尿病是威胁福州市居民的主要死亡原因之一,且对男性 影响更大,应采取综合性干预措施,预防或减少糖尿病的发生。
Abstract:
Objective To explore the death causes of patients with diabetes mellitus in Fuzhou city for providing the basis of developing the prevention and control strategies of diabetes mellitus. Methods Through the household survey, the death causes of diabetes mellitus in Fuzhou city from2011-2016 were overall analyzed according to the standard and method of ICD-10. Results Totally5 197 pa-tients with diabetes mellitus had been dead in Fuzhou city from 2011 to 2016, with the average mor-tality rate of 17.52 per 100,000 and the mortality rate in females(18.77 per 100,000)was higher than that in males (16.33 per 100,000), and the difference was highly significant(Z=5.019,P< 0.001). Urban mortality (28.75 per 100,000)is higher than the countryside (10.43 per 100,000), and the difference was highly significant(Z=36.719,P<0.001). People above 45 years old were the high-risk group. The rate of years of potential life lost(YPLL)and working years of potential lost rate(WYPLL)among residents was 0.39‰and 0.60‰, respectively. The influences of potential life lost and potential working life lost from diabetes mellitus in males were more conspicuous than those in females, which the rate of YPLL and WYPLL in males was 0.43‰and 0.65‰, respectively, while those in females was0.36‰and 0.54‰, respectively, and the difference were highly significant (Z=9.652, 12.229, P<0.001). Conclusion Diabetes mellitus is the main cause of death among resi-dents in Fuzhou city, and the influence on males is more conspicuous. It is necessary to carry out the comprehensive intervention measures to prevent and decrease the incidence of diabetes mellitus.
