摘要:
目的探讨老年2 型糖尿病(T2MD) 患者轻度认知功能障碍(MCI) 与其糖化血红蛋白(HbA1c) 水平 的关系。方法将2015 年5 月至2016 年12 月于广州市花都区人民医院神经内科收治的58 例存在MCI 的老年 T2MD 患者作为病例组,选择同期认知功能正常的56 例老年T2MD 患者作为对照组。用蒙特利尔认知评估量表 (MoCA) 对所有研究对象进行评分,评估其认知功能,并测定其HbA1c。观察并比较两组患者的认知功能与 HbA1c水平。结果病例组和对照组MoCA 总分分别为(19.43±2.30) 分和(26.58±1.74) 分,HbA1c水平分别 为(9.52±2.60) %和(5.35±1.37) %,差异均有统计学意义(P<0.05)。病例组和对照组中HbA1c水平异常升高 的比率分别为55.17%和21.43%, 病例组HbA1c 水平异常升高者明显增多(OR 值=- 2.35, χ2=13.69, P< 0.05)。结论老年T2MD 并MCI患者更易发生血中HbA1c水平异常升高。
Abstract:
Objective To explore the relationship between mild cognitive impairment (MCI) in elderly patients with type 2 diabetes mellitus (T2MD) and glycosylated hemoglobin A1c (HbA1c) level. Methods All research subjects were recruited from the Neurologic Department of Guangzhou Huadu District People’s Hospital during May 2015 to December 2016. The case group included 58 elderly patients with T2MD who had MCI. The control group included 56 elderly patients with T2MD whose cognitive function was normal. The Montreal Cognitive Assessme (MoCA) was administered to all subjects to test their cognitive fonction. The HbA1c level of all subjects were assayed. The cognitive functions and HbA1c levels of two groups were compared. Results Compared with the control group, the case group had significantly lower score in MoCA [ (19.43±2.304) vs. (26.58±1.736) , P<0.05], and the case group had significantly higher level of HbA1c [ (9.52±2.604) % vs. (5.35±1.372) %, P< 0.05]. The prevalence of abnormal level of HbA1c in the case group was higher than that in the control group (55.17% vs. 21.43%, OR=-2.35, χ2 =13.69,P<0.05) . Conclusions The elderly patients with T2DM who have MCI are more likely to have abnormal elevated HbA1c levels in their blood.
