摘要:
目的探讨慢性阻塞性肺疾病(COPD) 患者外周血脂肪炎症因子Apelin、一氧化氮合酶(NOS)、一氧 化氮(NO) 水平与COPD 所致肺心病的关系,为临床诊治提供参考。方法选择2015 年1 月至2016 年12 月 COPD 合并肺动脉高压、肺心病,未出现心衰患者30 例(肺动脉高压组),单纯COPD 患者30 例(COPD 组), COPD 合并心衰患者30例(心衰组),健康体检者30例(对照组) 作为研究对象。超声心动图测定左心室射血分 数(LVEF),肺功能检测仪检查第1 秒用力呼气容积(FEV1) /用力肺活量(FVC),抽取静脉血进行Apelin、 NO、NOS 水平测定,比较四组间差异。结果与对照组比较,肺动脉高压组、COPD 组、心衰组FEV1/ FVC、LVEF 均明显降低,且肺动脉高压组、心衰组低于COPD 组(P<0.05);COPD 组Apelin、NOS、NO 水 平与对照组比较差异均有统计学意义(P<0.05);肺动脉高压组和心衰组Apelin、NOS、NO 水平较对照组和 COPD 组降低,差异有统计学意义(P<0.05)。肺动脉压与Apelin、NO、NOS 存在负相关性。结论Apelin、 NOS、NO 可能是肺循环稳态的重要因素。
Abstract:
Objective To determine the value of Apelin, nitric oxide synthase (NOS) , nitric oxide (NO) in peripheral blood on the chronic obstructive pulmonary disease (COPD) induced pulmonary heart disease. Methods From January 2015 to December 2016, 30 COPD patients (COPD group) , 30 COPD patients with pulmonary hypertension or pulmonary heart disease (pulmonary hypertension group) , 30 COPD combined with heart failure paitents (heart failure group) , 30 healthy subjects (control group) were collected. Left ventricular ejection fraction (LVEF) was measured by echocardiography; FEV1/FVC was measured by pulmonary function diagram tester; Apelin, NO, NOS levels in venous blood samples were determinated and compared in different groups. Results Compared with the control group, the levels of FEV1/FVC and LVEF in COPD group, pulmonary hypertension group, heart failure group were significantly decreased, and pulmonary hypertension group and heart failure group were lower than those in the COPD group (P<0.05) . Apelin, NOS, NO levels in COPD group were lower than those in the control group (P<0.05) , and those in pulmonary hypertension group and heart failure group were lower than those in the control group and COPD group (P< 0.05) . The pulmonary artery pressure was correlated with Apelin, NO, NOS (P<0.05) . ConclusionApelin, NOS and NO may be the important factors of homeostasis of pulmonary circulation in COPD patients.
