摘要:
目的探讨使用动态增强扫描(DCE-MRI) 及扩散加权成像(DWI) 对前列腺良恶性疾病的鉴别。方 法随机选取2014 年6 月至2016 年12 月贺州市中医医院放射科确诊为前列腺增生(BPH) 的患者78例为 前列腺增生组;选取同期确诊为前列腺癌患者78例为前列腺癌组;另外选取同期来检查并确诊为健康受检者30例 为对照组。对这三组受检者使用DCE-MRI及DWI,分析其DCE-MRI SI-T 曲线,测量峰值时间(Tmax),峰 值强度(Sipeak)、强化率(R),并对比DCE-MRI参数和表观扩散系数(ADC) 值。分析联合诊断在前列腺良 恶性疾病中的应用价值。结果Tmax比较,正常组>前列腺增生组>前列腺癌组;SImax%比较,前列腺增生组和前 列腺癌组>正常组;R 比较,前列腺癌组>列腺增生组>正常组;ADC 比较,前列腺癌组<正常组<前列腺增生 组,差异有统计学意义(P<0.05)。DCE-MRI 联合DWI 诊断前列腺良恶性疾病的敏感性为92.31%、特异性为 87.18%、阳性预测值为87.80%、阴性预测值为91.89%、有效性为89.74%。结论DCE-MRI及DWI在前列腺良 恶性疾病性质的临床诊断与鉴别中具有较高的应用价值,能够有效评估前列腺疾病的生物学特性。
Abstract:
Objective To investigate the differential diagnosis of benign and malignant prostate diseases by dynamic contrast- enhanced MRI (DCE- MRI) and diffusion weighted imaging (DWI). Methods From June 2014 to December 2016, 78 patients with prostate hyperplasia (BPH), 78 patients with prostate cancer and 30 healthy cases in Department of Radiology, Hezhou Traditional Chinese Medicine Hospital were studied by DCE-MRI and DWI. SI-T curve, the peak time (Tmax), peak intensity (Sipeak) and enhancement rate (R) were measured and DCE- MRI parameters and diffusion coefficient (ADC) value were compared in these groups. The value of combined diagnosis methods in prostate benign and malignant disease was analyzed. Results Sequence of Tmax from high to low was normal group, BPH group and prostate cancer group; levels of SImax% in BPH group and prostate cancer group were higher than that in normal group; sequence of R from high to low was prostate cancer group, BPH group and normal group; sequence of ADC from high to low was prostate cancer group, normal group, BPH group (P<0.05). The sensitivity of DCE-MRI combined with DWI in diagnosing benign and malignant prostate disease was 89.74% ; the specificity was 87.18% ; the positive predictive value was 84.34% ; the negative predictive value was 89.47% ; the validity was 88.46% . ConclusionDCE- MRI and DWI have high application value in the clinical diagnosis and identification of benign and malignant prostatic diseases, and they can effectively evaluate the biological characteristics of prostate diseases.
