摘要:
目的探讨上肢淋巴水肿对乳腺癌术后患者的心理和生活质量的影响。方法选取中国人民解放军第 三○三医院普通外科2012 年1 月至2016 年1 月诊断为单侧乳腺癌且行乳腺癌根治术或改良根治术的患者212 例， 以48 例术后1 年内发生上肢淋巴水肿者作为病例组，采用病例-对照研究的方法，根据年龄、体质量指数、肿瘤 分期、手术方式对同期患者进行1∶1 匹配选取48 例作为对照组，分别采用汉密尔顿焦虑量表、生活质量评分量 表（QLQ-c30） 对比分析两组术后随访1年时心理状况和生活质量。结果两组患者的汉密尔顿焦虑量表评分 比较，差异有统计学意义（Z=－2.202，P=0.028）。病例组确定有焦虑（总分≥14 分） 的患者占41.67% （20/ 48），显著高于对照组的22.92% （11/48）。病例组患者的躯体功能、情绪功能和社会功能及疲倦、疼痛和失眠症 状的QLQ-C30评分均显著高于对照组，差异有统计学意义（均P＜0.05）。结论上肢淋巴水肿明显影响乳腺 癌术后患者的心理状况和生活质量。
