摘要:
目的探讨控制原发性高血压患者危险因素后血浆致动脉硬化指数(atherogenic index of plasma, AIP) 的变化及意义。方法收集上海市闸北区市北医院门诊及住院的原发性高血压病患者201 例,给合理的生 活方式指导和严格的血压、血脂、血糖等危险因素控制,随访观察12个月,记录年龄、性别、控制前后血压、血 脂、颈动脉内膜中层厚度(IMT)、血糖等检查结果。根据甘油三酯(TG) 和高密度脂蛋白胆固醇(HCL-C) 检测结果计算AIP。比较控制危险因素前后AIP 变化与脉搏波传导速度(pulse wave velocity,PWV)、血压、 血糖、血脂、IMT 变化的相关性。采用SPSS17.0 统计软件进行数据处理,计量资料采用均数±标准差( xˉ± s )表 示,控制前后均数比较采用t 检验,控制危险因素AIP 值变化与各项指标的相关性采用Pearson 相关分析,以 AIP 值变化以logistic回归分析其影响因素,以P<0.05 为差异有统计学意义。结果①控制危险因素前后AIP 变 化及各项指标均存在显著性差异(P<0.01)。②在控制其他相关变量的影响条件下,AIP 变化分别与PWV、总胆 固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、TG、IMT 的变化存在正相关(P<0.05),与HCL-C 存在负相 关(P<0.05)。结论控制危险因素后原发性高血压患者AIP 下降,可以作为动脉硬化改善的评价指标。
Abstract:
Objective To investigate the change and significance of atherogenic index of plasma (AIP) in patients with primary hypertension after controlling its risk factors. Methods Totally 201 cases of patients with primary hypertension were collected from Shibei hospital of Zhabei district. They all received reasonable lifestyle guidance and strict blood pressure, blood lipids, blood glucose control, and they were followed up for 12 months. Mean gender, blood pressure, blood lipid, carotid intima- media thickness, blood glucose and so on. AIP was calculated according to the results of triglyceride (TG) and high-density lipoprotein cholesterol (HDL- C). AIP and pulse wave velocity (PWV), blood pressure, blood glucose, blood lipid and carotid intima media thickness (IMT) in patients were compared before and after controlling the related risk factor. SPSS17.0 statistical system was used to analyze data, xˉ± s signified measurement data. Mean was compared by t test before and after controlling the risk factors. Pearson correlation analysis was used to analyze the relation among AIP and other observing indexes. Influence factors of AIP was analyzed bylogistic regressionanalysis. P<0.05 was significant difference. Results ① There were significant differences in AIP and all other indexes before and after controlling risk factors (P<0.01).② Under controlling the other variables, AIP was positively related with PWV, total cholesterol (TC), low density lipoprotein cholesterol (LDL- C), TG, IMT, and negatively related with HDL- C (P<0.05). Conclusions AIP decreases in patients with essential hypertension after controlling risk factors, and it can act as improvement indicators of arteriosclerosis.
