目的 分析2008-2012年内蒙古自治区男男性行为者(men who have sex with men,MSM)艾滋病疫情特征。方法 分析2008年1月～2012年8月中国艾滋病病例报告系统中男男性传播病例的三间分布(时间、人群、地区)及哨点监测系统中MSM的HIV抗体检出率及安全套使用率。结果 自2008年1月～2012年8月内蒙古自治区艾滋病传播途径构成情况发生显著变化,其主要表现为性传播逐年上升,构成比从38.2%上升至81.2%,其中以男男同性传播上升尤为明显,从20.1%上升至41.1%,5年平均增长率为19.5%; 与其他传播途径相比,男男同性传播感染的人群具有25～34岁为主、文化程度较高、未婚、蒙古族、家政或商业服务或学生占相当高的比例、流动性强、以城市为主、性病门诊及其他就诊者检出比例较高的特征; 地区分布差异较大,在内蒙古东部报告病例中所占比例最大,非边境地区报告的男男同性感染人群高于边境地区; 哨点监测HIV抗体阳性检出率提高幅度较明显,2012年为2008年的28.3倍; 最近一次与男性发生性行为安全套使用率在近5年内的中位数为77.9%。结论 MSM人群中艾滋病防控工作,要结合当地情况,宣传教育、安全套使用等干预工作要有针对性。
Abstract:
Objective To analyze the epidemic characteristics of HIV/AIDS among men who have sex with men(MSM)from 2008 to 2012 in inner Mongolia. Methods Through the network reporting information system.The distribution of MSMtransmission was analyzed.Through sentinel surveillance system from 2008 to 2012,data was gathered and analyzed for HIV/AIDS positive rates and condom use to describe the epidemic characteristics of HIV/AIDS among MSM. Results From January 2008 to August 2012,Inner Mongolia autonomous region transmission way constitution significantly changed,its main performance for sexually transmitted is rising year by year,constituent ratio from 38.2% to 81.2%,among them with other gay propagation rise is particularly obvious,increased from 20.1% to 41.1%,5 years has grown at an average rate of 19.5%.Compared with other transmission way,male-male gay spread the infection of the crowd has the following characteristics:25 to 34 years old is given priority to,high degree of culture,unmarried,Mongolian,domestic/commercial service and students accounted for the proportion of very high,liquidity,city is given priority to,venereal outpatient/other issued detection such as higher percentage.Regional distribution differences,in eastern Inner Mongolia case report and the proportion of the largest,in the border regions report with other gay infections than border area.Shao point monitoring HIV antibody positive detection rate increase was more obvious in 2012,28.3 times that of 2008.“A recent and male sex condom usage” in the last 5 years of median77.9%. Conclusions The prevention and control of HIV/AIDS intervention among MSM population work,in the light of the local conditions,publicity education and condom use intervention work to be targeted.
