摘要:
目的 通过痰标本抗酸杆菌涂片镜检和培养结果的分析,探讨如何提高抗酸杆菌检出率。方法 收集1 171份痰标本,采用萋尼氏染色进行涂片镜检和分枝杆菌培养,并对结果进行统计学分析。结果 涂片阳性率13.0%,其中初诊患者痰涂片阳性率15.4%,复诊患者痰涂片阳性率6.8%; 培阳率16.0%,其中初诊患者培阳率20.1%,复诊患者培阳率5.7%; 初诊患者培阳率与痰涂片阳性率之间的差异有统计学意义(χ2=6.22,P<0.05),复诊患者痰涂片阳性率与培阳率之间的差异无统计学意义(χ2=0.41,P>0.05)。初诊患者痰涂片阴性培阳率为5.3%,痰涂片阳性培阴率为0.6%; 复诊患者涂阴培阳率为3.0%,痰涂片阳性培阴率为4.1%,复诊患者痰涂片阳性培阴率明显高于初诊患者,差异有统计学意义(χ2=57.97,P<0.01)。培养和涂片联合检出阳性率17.6%,其中初诊患者阳性率20.7%,复诊患者阳性率9.8%。初诊患者菌落出现平均时间为18 d,复诊患者平均时间29 d。结论 痰涂片总的培养阳性检出率优于涂片检查,两者联合应用的阳性检出率优于其中任何一种方法,初诊病人的痰检阳性率及培养菌落出现平均时间均优于复诊患者。
Abstract:
Objective To analyze the results of smear test and culture of acid-fast bacilli on sputum samples,and to explore how to improve the detection rate of acid-fast bacilli. Methods Ziehl-Neelsen staining and mycobacteria culture were carried out on 1171 collected sputum samples.The results of the test were analyzed statistically. Results The total smear-positive rate was 13.0%,with 15.4% for first visit samples and 6.8% for return visit samples; The total culture-positive rate was 16.0%,with 20.1% for first visit samples and 5.7% for return visit samples; There was statistically significant difference between the smear-positive rate and the culture-positive rate of the first visit samples(χ2=6.22,P<0.05),but that of return visit samples was not statistically significant(χ2=0.41,P>0.05).The rate of smear-negative culture-positive and smear-positive culture-negative for the first visit samples was 5.3% and 0.6%,respectively; The rate of smear-negative culture-positive and smear-positive culture-negative for the return visit samples was 3.0% and 4.1%,respectively.The rate of smear-positive culture-negative for the return visit samples was higher than that of first visit samples,and the difference was statistically significant(χ2=57.97,P<0.01).The total smear and culture positive rate was 17.6%,with 20.7% for first visit samples,and 9.8% for return visit samples.It took an average of 18 days for bacteria colonies of first visit samples to emerge,while 29 days were needed for return visit samples,The difference between them was statistically significant. Conclusions The positive rate of sputum culture was much better than that of smear test.When using of the two methods together,the positive rate was better than the results provided by either of the two methods.The positive rate and average emerging time of bacteria colonies of the first visit samples were better than those of the return visit samples.
